Why the name change? It's pretty simple. Liberty Mountain Medical Group was formed by members of Collaborative Health Partners in affiliation with Liberty University College of Osteopathic Medicine. Liberty Mountain has served as a multi-specialty, academic medical practice that works in a team-based approach with nurses, allied health staff, medical students and residents to provide outstanding care to the general public. The entity has grown significantly—there has been an increase in the popularity of its specialized practices, which has expanded to include additional patient-facing services such as sports medicine and pediatric medicine.

Rehab Associates of Central Virginia joins Collaborative Health Partners in offering Direct Care to Liberty University employees. Through a contract with Rehab Associates, an outpatient physical therapy provider, Liberty will offer direct access to physical therapists, providing for quicker treatments to limit pain and return to function, all while saving both the employee and employer time and money.

Privia’s ACOs (collectively “Privia Quality Network” (PQN)) achieved shared savings of $99.9 million through the Medicare Shared Savings Program (MSSP). Together, Privia Health ACOs provided high-value, cost-efficient care to more than 112,000 Medicare beneficiaries in 2021.
Liberty Health Services, a Virginia limited liability company, is announcing today the acquisition of Collaborative Health Partners (CHP), a healthcare management services organization with a clinically integrated network of providers headquartered in Central Virginia. This acquisition will expand CHP’s ability to grow and further support the primary care, specialty care, and ancillary service offerings of its current clients, CVFP Medical Group and Liberty Mountain Medical Group.
