Why the name change? It's pretty simple. Liberty Mountain Medical Group was formed by members of Collaborative Health Partners in affiliation with Liberty University College of Osteopathic Medicine. Liberty Mountain has served as a multi-specialty, academic medical practice that works in a team-based approach with nurses, allied health staff, medical students and residents to provide outstanding care to the general public. The entity has grown significantly—there has been an increase in the popularity of its specialized practices, which has expanded to include additional patient-facing services such as sports medicine and pediatric medicine.

Liberty Mountain Medical Group will officially take on the Collaborative Health Partners name, effective July 1, 2023.

Collaborative Health currently supports two other healthcare entities, CVFP Medical Group and Walk-In-Care, and brings together several local healthcare partners to provide services to area employers. These services include direct primary care, physical and occupational therapy, health screenings, and more.
