We are thrilled to announce that Collaborative Health Partners (CHP) is expanding its direct primary care services to the public! Since 2015, CHP has been providing exceptional care to local employer groups through its partner, CVFP Medical Group, and now, we are extending our services to offer even better access to primary care for our growing community.
At CHP, we believe everyone deserves high-quality healthcare that is convenient and accessible. By expanding our direct primary care services, we aim to make healthcare more accessible to individuals and families in our community.
Currently, our Collaborative Health DPC clinic operates out of our CVFP Forest location, where our dedicated team of DPC providers, Dr. Joanna Thomas and Brandon Lipscomb, FNP are ready to serve you.
